Back to work ... got some good things done
Good morning. This is me. We’re at work and it is about 8:30 am. I’ve been here about an hour. We read through our mail and then checked our on-line things. This has gotten me up to here. Hmm, I read part of a TIME article on birth order, but I didn’t think too much of the story. It seemed pretty irrelevant. We had a small blip on the way to work. We were rolling out onto a street, because we couldn’t see around the parked car. A car came around the corner fast to turn in front of me to stop, but then the woman pulled up aside me and was like yelling, showing an angry ugly face, and shaking her hand at me. It was like … what the hell is all that? We pulled up so she was out of vision, shrugged our shoulders and then pulled onto the street proper. The whole thing was pretty shocking not that I had gotten in her way, but that someone could want to share that much ugliness. It was pretty terrible. We called Rich and he said to forget about it, so we’re going to try and do that now. She was really unbelievably ugly though. My eyes just squinched like what? What? What are your trying to act out? HMM??
Ok, enough of that. I did get to work and we got here on time. Sister Theresa opened the door, but two clients and another staff were coming in at the same time, so she didn’t say anything to me personally. She just said to the group that it looked like everyone was here.
It’s about 8:45 am now. I just had a visit from Sister. She still had her coat on from being out at church. She has a few minutes between that and the chapel service that she gives for the group.
Sr. sat down in the chair and asked me what had happened … we knew that the first thing she was going to say was that we didn’t call in. We apologized sincerely for that and it wasn’t much past a few moments and we started to cry. We told her that we’d gotten pretty much pinned down to the thoughts that our body was falling apart and that we are going to die. We told her about thinking of my father and how he fell apart and that we were doing the same thing, but we were 20 years younger. I told her that everything is going so well as to friendships, job, and kids, but that makes us feel bad because we were going to lose it all. She asked also about Dr. Marvin. We admitted that we hadn’t gone in … we told her we weren’t able to leave the house for work, therapy, Dr. Marvin and the medicine. She was very nice about it, but it still hurt. It hurts now. Not anything to do with Sr. Theresa directly toward me, but that we feel we’re failing her.
She emphasized that this hadn’t happened for a long time, and that it didn’t do any good to think about it and then she asked what our advice would be to someone else coming in with it. We said that we’d take it a day at a time. There was more, but I have to stop because we’re getting all bad. We just want to note that we had the impression that Sr. Theresa cared what happened to us. Right now we want to dissociate for a bit. Maybe go back to facebook. We need to quiet down. I wish we’d remembered to bring some coffee, but we were worrying about where our purse was. It turned out to be in the car, but until we could get down there we just needed to get out of the house and look. Fortunately, it was there.
Ok, ok … need to calm down. Facebook, remember? I think one of the reasons we keep going back there was that it had that nice picture of us in the dark. Thinking of wanting to be that person made us feel better. We had told Sr. that everything seems unable to be fixed and that was leaving us feel helpless. Ok, shh… that’s enough … let’s go to the other page, k?
Ok, that’s it … it’s now a quarter after nine and we’re feeling a bit more calmed down. We did a couple more searches and we updated our profile. I wanted to include that we were part of the AOL Journal and Blogger world. I’m not sure how Theresa found us, but we’re guessing that she’s still in touch with Vince and Vince told her we were a part of it too. Maybe she had mercy on us for having so few friends. On the other side of the coin, she is flooded with friends. Most seem connected from her University that she teaches at.
Whoops, just took a side break. Rich came into the office and stated that he had people from another agency to look at the place. He gave me the opportunity to explain the program at St. Rose Center and then go into the groups. I explained how things are coordinated together using the areas of staff training, CARF, the groups, etc. Funny how we can go from zero to sixty if need be. It went pretty well. Sr., Rich, and us talked to the two gentleman, and then they went on to have a tour of the building and I bowed out.
I’m still thinking a cup of coffee would be fine.
Ok, that’s not going to happen … we have to move on, k?
Yeeks … just saw some ants up on my computer desk. We had the spray can from before so we outlined the desk. That usually does it. I think its time for the ORKIN guy to come again. I wonder if his stuff was swept away when we had someone come in to sweep the dead ants away. Unfortunately, now we have to put up with the smell.
*Sigh*
Ok, that’s enough excitement for the day. I wonder if there is anything else we need to think through before we consider getting actual work done. It’s now about 10 am. There’s always this process we go through trying to remember what our work is. We’ve got some things out from one of our client’s goals. I wonder what we were doing there. Hmm, that’s right … we had some kind of list we were working from … let’s look for that.
Ok, stopped for the orange break and washroom and to get the flash drive out and plugged in. What we found on our recent docs was a “Hot List of Things to Do.” I think this is probably important. Better check what date it is too. Hmm, turns out to be the 22nd. I wonder what day that last staffing was on? Ok, we’re good there … last staffing is on Wednesday the 24th. We can do that tomorrow, but better get in that person’s Qnotes. Where are we there? Ok, good … got in at least through July. We need to get and enter August and September … let’s see if we have those documents. We need these things done so we can document progress on the agenda. I think that we have to talk to the DSP too. Better arrange that sometime today. I think she is with Group 2 today. Everyone here is cross-trained. Ok, so maybe first we finish up the objectives for the first client? Let’s see where we left off there.
Ok, figured that out pretty much. We’ve completed her annual report and we’ve written out the first couple of paragraphs on the Wellness goal. Basically, this is what it says …
Wellness Chart (Personal Living Skills) So and So will record and graph 7 items (shower, hair brushing, tooth brushing, clean clothes, sleep and during the center’s time - water and exercise) looking toward progress in each. So and So’s goal will be complete in one year, or when she has completed all steps at 100% effectiveness for a total working score of 8.
So and So’s long-term goal and technique: So and So will have a daily chart she needs to complete. It will ask her if she has taken a shower that morning or during the night, if she brushed her hair and teeth before coming to the center, and if she put on clean clothing. So and So has to circle the correct response and have it checked by the DSP. So and So’s uncle will give So and So a timer to start and stop when she gets up and goes to bed and that number will be written on a 3x5 card and recorded on the chart. So and So will also have a water bottle on her desk. Each time the water bottle is emptied, So and So will make a circle on her chart, and last So and So, will ask her teacher if she exercised at a low, medium, or high rate and this will also be charted. The charts will be given to the QMRP at the end of the month with the DSP Goal Data Sheet and it will then be tallied and recorded. So and So’s working score will be the steps (Day of the week/optimal number of points) divided by the total number (85 points) times the number of trials per month.
Steps:
1) So and So will take a shower each day of the week
a. Monday (1 point)
b. Tuesday (2 points)
c. Wednesday (3 points)
d. Thursday (4 points)
e. Friday (5 points)
2) So and So will brush her hair each day of the week
a. Monday (1 point)
b. Tuesday (2 points)
c. Wednesday (3 points)
d. Thursday (4 points)
e. Friday (5 points)
3) So and So will brush her teeth each day of the week
a. Monday (1 point)
b. Tuesday (2 points)
c. Wednesday (3 points)
d. Thursday (4 points)
e. Friday (5 points)
4) So and So will put on clean clothing each day of the week
a. Monday (1 point)
b. Tuesday (2 points)
c. Wednesday (3 points)
d. Thursday (4 points)
e. Friday (5 points)
5) So and So will sleep for the optimal number of 8 hours (6 points) (or optionally 9 hours (5 points), 7 hours (4 points), 6 hours (3 points), 5 hours (2 points), or 4 hours (1 point)).
a. Monday (up to 6 points)
b. Tuesday (up to 6 points)
c. Wednesday (up to 6 points)
d. Thursday (up to 6 points)
e. Friday (up to 6 points)
6) So and So will drink appropriate amounts of water (4 bottles (4 points), 3 bottles (3 points), 2 bottles (2 points), or 1 bottle (1 bottle).)
a. Monday (up to 4 points)
b. Tuesday (up to 4 points)
c. Wednesday (up to 4 points)
d. Thursday (up to 4 points)
e. Friday (up to 4 points)
7) So and So will work toward the optimal level of effort while exercising (High level (3 points), medium level (2 points), or low level (1 point)).
a. Monday (up to 3 points)
b. Tuesday (up to 3 points)
c. Wednesday (up to 3 points)
d. Thursday (up to 3 points)
e. Friday (up to 3 points)
Ok, ok … that’s good … we’re inserting a chart to go with that.
Hmm, pretty good? This is for the first of this person’s three goals. I like the way this chart came out … We’re hoping that it works. We just need to keep the client motivated.
Ok, now we have to look at goal #2. I can’t do the steps on this one because of all the client’s names, we’ll catch you on the last one.
Goal #2 – Social Skills (Social and Communication Skills) So and So will interview each of her peers at St. Rose Center and note in a journal 3 interests of each. So and so’s goal will be complete in one year, or when she has completed all steps at 100% effectiveness for a total working score of 8.
So and So’s long-term goal and technique: So and So will interview each of her peer’s and each of the staff and ask them to name 3 things that are important to them. She could ask “Can you name three things that are important to you.” She will tape record the interview. So and So’s DSP will record on paper the three interests and So and So will copy them to a journal-like book. So and So’s working score will be the steps (Group/individuals in the group) divided by the total number (57 points) times the number of trials per month.
Hmm, ok this should be pretty easy. We’re going to need to make a goal sheet with everyone’s name on it from the center.
Ok, that goal is written …
Woohoo FINISHED that client! Her is her calendar.
Pretty cute … Hmm?
I love it. This was the third goal.
Goal #3 – Week days (Personal Living Skills) So and So will learn the days of the week by using a personally designed calendar and stickers to track the days she honors data collection from goal #1 . So and So’s goal will be complete in one year, or when she has completed all steps at 100% effectiveness for a total working score of 8.
So and So’s long-term goal and technique: So and So will be given a calendar that is special in that it has one calendar for her, and one calendar for her boyfriend. The calendar for her boyfriend is to inspire motivation, but will not be counted toward the points. So and So can have a sticker added to Michael’s calendar for the days that she talked to him. On the days that So and So completed her wellness chart (within 24 hours) she can mark the calendar date with a sticker. The weeks will be given 1 point each for the sake of counting, but will be added cumulatively each month. The DSP will simply mark an X on her goal data sheet on the days that So and So has marked her calendar and the QMRP will total scores. So and So will keep the calendars that are completed. So and So’s working score will be the steps (number of days per week/month) divided by the total number (52 points) times the number of trials per month.
1) So and So will mark with a sticker the days she has completed her wellness chart for October and be scored 20% of 1 point for each day of the week she has marked on the calendar for example.
a. Week 1 (1 point)
i. Monday (20%)

Ok, ok … that’s it … we’re done with it … You can tell that there is a lot of repetition, so it is about finding the parts that will work together. Like in this last part I had to figure out points per week and per day within that. This is really an easy thing to score after you figure out the system, but it is always tricky to teach the teacher before she panics. They get easily overwhelmed. Better to explain the task and then show her this set of documents.
Now its about 4:20 pm. We've got almost everything else ready to go. Just wanted to finish off here. There were a couple of Sr. interventions. I guess the new Q had a staffing and had a terrible presentation of materials. We talked to Sr. first - called in. And, then we talked to Candice. We didn't tell Candice that Sr. had already told us. So, then since she was pretty upset, we told her she should do something ... like yes, eat lunch!!! But, then she disappeared now for about an hour and a half. That meant that she's visiting with Holly. That also means she's not getting her work done.
We printed out a copy of the form she should have used. I don't understand her. Instead of asking or looking in the folders for the proper format, she went solo. And, then she said that she thought Sr. would want it cut down. Uh uh. Sr. wants what's done proper. Candice just ran out of time. She doesn't seem to think that all these extra trips and preoccupations from the computer are killing her. Sr. stated that she wanted me at the next annual. That means that I'm going to be backed up.
After the form was completed, I went back and added dozens of questions she should be asking for each of the major statements. It's a very good presentation. I wish Sr. would have allowed me more leeway with Candice. Now she's in a bundle because the staff that she should be asking the quesions of is away at staff training meetings out of the building. Maybe I should check my days. Hmm, that sounds about right. The DSP in training is only in the course through Thursday. Candice is still avoiding getting hold of the ISSA person to get down a meeting date, but it should be next week. I am leaving a note on Candice's desk saying that the other staff might be back on Friday.
I'm afraid that she's feeling too much pressure. When sister was back here asking me to cover something tomorrow, she told Candice that they will be meeting tomorrow to go over the meeting. They had both explained it as a disaster. Sr. didn't think Candice was well prepared, knowledgable, or had put forth much effort. She didn't. I was showed the report she handn't in it was of low quality. Candice thought that she had it under control and had enough information, and she blamed Sr. for telling her she didn't do well especially in front of parents and CSO person. So, I guess it was a bad deal.
I could have gone home a half an hour ago, but I sort of want to see Candice out. She is going to have to stop talking to Holly to go home. It's not a good idea that Holly is into this, but somehow Candice thinks to go to her for the parts past immediate need. I was glad she came here first, but disappointed she didn't get right to work to correct things. I don't like the idea that her emotional needs are getting covered by Holly. Sr. is down on Holly now, so neither are getting points for using the others time. And, Holly isn't a good advisor. She'll empathize, but then she'll just convert the time over to her talking on and on about herself, or her thoughts. Candice needs to deal with the problem.
I have to look at myself and time like this while I'm away from work, but I feel its different writing as a self-motivation tool to get back on task. Or, to conclude the day after the day is formally over. I can't say I haven't gone through what Candice is, but I went in to ask questions. She's gone in to gain "comfort." It's not a good idea to tell trainee don't worry about it, when that's exactly what she has to do. She has to figure out what went wrong. She's so cocky and think she doesn't need help, but she does ... Holly isn't going to help her. Holly just wants some reason to avoid work. It's like shoot ... get to a point and then move on!
Ok, speaking of, I see I'm just frustrating myself here. I've got 20 minutes before Candice formally gets off. She has to make up time. I could leave, or I could do the time and be nice to bring her to the bus station. It's just a little out of the way. I want to figure out where she is at after being with Holly and then get her back on track. Hmm, is there anything I could do to occupy myself? Hmm, there is a Qnote begging to be typed. That would take about 20 minutes. Maybe we'll go there. Ok, outta here ... see you soon.
