As far as medical concerns ...

Health Improvements noted:

No smoking - Stop Date July 25,2006; hypnotized
Rare drinking - At most 2 at one time about once every 2-3 months
No Drug problems - with the exception of acting out suicidally with sleeping pills 3-4 weeks ago (purchased, but not taken)
Physical Fitness - 45 min per day (1 week)
Diet - Approximately target of 1800 calories - concerned with nutritional balance (3 weeks) Starting weight (home scale) 312.4 now 297.8 = 14.6 pounds; had lost in 6 months previous 40 pounds then within 2-3 months gained all weight back
Support - Being very positively supported by family, friends, and co-workers

Physical Concerns:

Diabetis - Blood sugar - will need to know when to take it and good range, etc, fearful of its overall likey affect on an early death
Cholesterol - fasting tonight by 7 pm for test tomorrow morning
Sleep - About 7 hours on recliner - fairly good sleep; laying down sleep very bad; become sore, congested, unable to sleep through night toss and turn, snoring wakes self up Thyroid - Nothing new; don't understand this condition much just know it makes it hard to lose weight and my temperature easily either too hot or cold
Ulner Neropathy
Left: moderate distraction, easily tired and achy hands
Right: mild distraction, easily tired and achy hands
Arthritis: Was having difficult going up stairs and walking more than 5-6 min at normal pace, gym speed of 2.3 on treadmill for 6 minutes daily; bike speed of 5 at 7 minutes twice daily
Feet - dry, sore, tingly/numb around toes (especially small); feel sensitive to pressure (heels)
Vision - New tri-focals fine, trouble seeing in dark more threatening now; two car scrapes in two weeks time
Hearing - Having difficulty hearing words close range; words unclear and soft
Stomach - fine, just too big; often feel hungry; sometimes impulsive eating
Breathing - still easily out of breath; easily congested; yawn a lot during the day, especially when talking leads to a drowsy feeling; sense of low oxygen
Dizzy - frequently when standing; body shifts shifts
Memory - Having trouble with follow-through in establishing memory routines like notes and scheduling that would assist moderate impairment
Menopause - haven't had period for 3-4 months, very cranky, easily upset - wondering if this is menopause; we need to talk to someone so we know what to expect
Dentist - Have appointment; need to consider a partial if can be made affordable with dental insurance

Mental Concerns:

Multiplicity - Still working at having lost time with Dr. Marvin; changed to weekly this last month, trying to work through issues of self-regulation - focus on goals, but extremely difficult; spacey and uninterested to highly pleased and motivated

Depression - Last time suicidal maybe 3-4 weeks ago, but we've been doing well with depression since focusing on the fitness; biggest concern seems to be loss of Dr. Marvin - sense of nobody understanding us or changes we are going through; sense of losing any drive to do real work over a preference for not working at all; afraid of negative affect on people who are close, especially children and grandchildren

Anxiety - lost ability to concentrate on school - in process of obtaining medical leave; having trouble focusing on deadlines at work; at work working at about 40% effectiveness with less efficiency; fearful of the heart problems a close friend has had recently - heart/stroke, or fear of cancer (very bad loss of hair, thinning) heredity of poor health from problems with parents and grandparents; worried of Grandmother dying soon (98 years old); continue to be fearful of people being angry with us or not liking us

Obsessive/compulsive - Centered now on fitness goals; trouble with over-spending money that is to be used for bills and rent; still not getting mail or doing housework - repulsed by touching anything dirty, so the affect is not wanting to touch anything; all extra time continues to be spent at computer

OVERALL: I need help holding all this information together in a manner where it won't overwhelm me again. I've gone 8-10 months without any medical appointments, which started because we were upset thinking Dr. Albright had kept us purposely waiting 30-40 minutes to conclude last 2-3 minutes AND because we've only seem to have small windows of healthful opportunities before we go into months and months of total medical and mental disregard.
